If you experience or witness mistreatment while you are rotating at Stanford clinical sites, we encourage you to report it through one of the options.
1. Contact the Office of Medical Student Wellness (OMSW) directly by email. Only the OMSW members of the office will see this report. They will de-identify information about persons reported and present to the monthly Respectful Environment and Mistreatment Committee for review and action.
2. Report incidents (including by patient) via MedHub (Stanford.medhub.com) or directly in Qualtrics. Students can report at the time of the incident and not wait for the end-of-course evaluations. OMSW receives the report. Egregious incidents are handled immediately. Qualtrics links: clerkship, and mistreatment by patient incidents.
3. Complete a SAFE report through the hospital SAFE reporting system. Reports referencing student mistreatment are sent directly to OMSW. They are kept confidential and addressed quickly.
4. Report incidents to course directors and/or the Assistant Dean for Clerkship Education.
