If there is a clerkship available, visiting students will be sent an email of their acceptance by the notificiation date listed on the Clerkship Application & Period Dates page. The notification email will include the link to our Acceptance Form should you wish to accept the scheduled clerkship.
Waitlist notifications will be sent to those who we were unable to schedule.
Follow the acceptance form's instructions for providing requested information and accepting the scheduled clerkship. Use the upload feature at the end of the form to include the required information.
The Acceptance Form must be received by the deadline date listed on the acceptance notification.
Acceptance Form:
- Upload a copy of your mask fit certification to show that you have successfully passed the Respiratory Mask Fit Test within 12 months of the end of your scheduled clerkship. See dates below.
2024-25 Academic Year Mask Fit Test:
-Summer Quarter - no older than September 2023
-Autumn Quarter - no older than December 2023
-Winter Quarter - no older than March 2024
-Spring Quarter - no older than June 2024
The acceptance notification will include instructions for providing medical compliance information (i.e., for medical clearance). Read through the information carefully and prepare to provide the following proof of required immunizations and tests.:
DO NOT submit immunization information/documents with the Acceptance Form. The information must be submitted through the university's health portal.
-MMR (Measles, Mumps, Rubella): 2 doses of live vaccines or laboratory evidence of immunity
-Varicella: 2 doses of live vaccines or laboratory evidence of immunity
-Hepatitis B: 3 doess of vaccines, or laboratory evidence of immunity from vaccination
-Tdap (Tetanus + Diptheria + Pertussis): Tdap booster (booster without Pertussis is not compliant), must be within past 10 years from the last day of the quarter
-Flu vaccine: Required seasonally - October thru March
-Tuberculin testing - TB testing completed within 12 months prior to send of clerkship, with laboratory evidence of negative QuantiFERON Gold or T-spot blood test. PPD is not accepted.
-If history of a positive TB skin or blood test, a chest x-ray completed within 12 months of start of the clerkship accompanied by positive TB test results.
