Identifying Patients

For managers of clinics or practitioner groups who want to implement PEP:

  • Use your EMR to generate lists of continuity patients who
    • Are 65 to 85 years old
    • Have been seen within the prior 90 days and
    • Have a PHQ9 score within that time with score between 5 and 14
  • Sort the list by primary care practitioner (PCP)
  • For those PCPs with at least 3 eligible patients, send the PCPs the invitation to join PEP (included in smartphrase index) along with their specific patient lists
    • Ask PCPs to identify which patients might benefit
      • Reasons that patients may not benefit include:
        • Not comfortable with video visits
        • Not comfortable with electronic messaging
        • Not comfortable speaking and reading English
        • Dementia or cognitive impairment
        • In hospice care or at end of life

 

For individual practitioners, we encourage you to use your EMR to identify your continuity patients whom you have seen at least once in the prior 90 days and have a PHQ 9 score of 5 to 14. You can also use PEP on an as needed basis for patients whom you think would benefit.
