Intake Form

The Positive Experience Program (P.E.P.) consists of 3 individual, weekly appointments, 20-30 min duration, with your patients who score as mildly to moderately depressed using PHQ9. It is grounded in 2 theoretical orientations:

1. Cognitive/ behavioral therapy – specifically, behavioral activation, which has been shown to be an effective stand-alone treatment for mild to moderate depression in patients across the life span, and

2. The “Tiny Habits”© approach which is a highly effective way to teach people to create new habits by using a set of specific techniques.

Techniques from these 2 approaches have been blended into a single program. Each of the 3 sessions is fully scripted. As the provider, you would read the scripts which ensures fidelity to the treatment modality. There are handouts and assignments to reinforce learning which are available as templates. Ideally, each patient needs to commit to having a visit with you once per week for three weeks.

Please complete the intake form below. Upon completion, you will be automatically redirected to access to the PEP materials.
