Diagnosis of coronary artery disease using coronary CT angiography with AI-enabled quantitation of coronary plaque burden and lesion-specific coronary ischemia is used to guide management of patients with suspected CAD. Applying this diagnostic strategy to PAD patients with no cardiac symptoms reveals a high prevalence of asymptomatic (silent) coronary ischemia, which is a marker for risk of sudden cardiac death or acute MI. Elective, ischemia-targeted coronary revascularization has been shown to reduce death and MI and improve long-term survival following carotid or peripheral revascularization procedures. Randomized trials in carotid and peripheral vascular patients are currently underway to test this hypothesis. .
