- Immunizations & other school entry requirements
- Patient concerns
- Brief mental health screen (PHQ or Distress Thermometer)
- SCH Epic chart and screening lab orders (see SCH Epic Order Set)
- Social work and/or resource coordinator visit
- Brief medical history and exam

Other elements of well visit as time allows
Schedule follow-up visit in 1 month
