Students must be in good academic standing and, at the time of the elective clerkship, will be registered in their 4th or final year in an MD or DO degree program at a school accredited by the Liaison Committee on Medical Education or Education Department of the American Osteopathic Association.

Students must have completed core clinical clerkships in Medicine, Pediatrics, and Surgery.

Students Must Have Completed and Must Provide Proof of Required Immunizations and/or Tests for Medical Clearance:

If students who are participating in a clerkship that takes place at a Veterans Administration (VA) hospital site, will be required to comply with the following requirements set by the VA:

• Evidence or self-certification that they are physically and mentally fit to perform the essential functions of the training program
• Evidence or self-certification of up-to-date vaccinations for healthcare workers as recommended by Centers for Disease Control (CDC) and VA
• Evidence of tuberculosis screening and testing per CDC health care personnel guidelines
• Identification documents to meet VA security requirements
• Results of screening against the Health and Human Services’ List of Excluded Individuals and Entities (LEIE)
• Trainees who were born male and who are US citizens, immigrants to the US, or are otherwise required by law to register, have registered with the Selective Service System
• Trainees who are non-US citizens:

Appropriate documents can be provided and could include permanent resident card, employment authorization document Form I-766, visas: J-1, J‑2, H-1B, H-4, E-3, or DS-2019

Have current immigrant, non-immigrant, exchange visitor or other documentation stating that they are eligible to live and work in the US

Have been issued a US social security number

NOTE: Students are required to have a VA PIV badge to rotate at the VA.  If students do not have an active PIV from their home institution and need to begin the process to obtain the PIV for the Stanford rotation, it can take up to 4 months to complete. 

Students must have passed and provide documentation of a Respiratory Mask Fit Test/Certification within 12 months prior to the end of the academic quarter. The test includes a measurement for mask size. 

• Stanford University Hospital and Clinics stock 3M 1860 or 1860S type N95 disposable respirators.
  

Students must be covered by malpractice/liability insurance at a minimum level of $1,000,000 primary/$3,000,000 aggregate by the home school.  If the home school does not meet our minimum coverage, documentation of supplemental insurance will be required to fulfill our requirements. 

Students must be covered by personal health insurance during the time they will be participating in the elective clerkship.

Students must have completed the criminal background check within 2 years prior to the end of the clerkship. 

Any basic criminal background check is acceptable.  Students have used online services if their home institution does not assist in providing one.      

Bloodborne Pathogens Training:

Students must have completed the training within 12 months prior to the end date of their schedelued clerkship period.

Safety & Emergency Preparedness Training:

Students must have completed the training within 12 months prior to the end date of their scheduled clerkship period.

•  It is acceptable if the home institution combines training in bloodborne pathogens and safety measures.  
• Training topics include: General Safety, IIPP, and Emergency Preparedness covers practices and procedures for preventing employee injury or illness from potential workplace hazards.  Appropriate for all employees and students in administrative and other work environments.
• Biosafety covers the safe use, handling, and disposal of biohazardous agents, and reviews emergency procedures for occupational injuries or biological material spills.
   

Basic Life Suport and/or Advanced Cardiac Life Support Training:

Students must have completed the training(s) within 2 years prior to the end date of their scheduled clerkship period.

The Stanford University Privacy Office requires visitors, including visiting students, to complete Stanford's HIPAA/Protecting Patient Privacy online training.  

After students create their SUNet ID and the account is active, an email notification will be sent to the students' Stanford email address with instructions on how to complete the training. 

Students must have passed Step 1 of the United States Medical Licensing Examination or the National Board of Osteopathic Medical Examiners certifying exam.

Instructions for paying the fee will be provided after students accept the scheduled elective clerkship.  

The non-refundable application fee of $200 must be received to complete the application.
