How To Apply to Become a Visiting Student

For the formal Application Form:

1. Have the Dean's Statement Form completed and signed by an authorized person from your school's Dean's Office, Student Affairs, or Registrar’s Office.
   
   
2. Have a copy of the pre-approval that was obtained.  This will usually be an email message from the clerkship department.  Save the email as a PDF file to upload to our application.  A pre-approval is required for each different clerkship being requested.   
   
   NOTE: The pre-approval process is conducted by the individual clerkship departments.  Therefore, please contact them directly if you have questions on obtaining the pre-approval.  Contact information can be found on our Course Bulletin by searching for the desired clerkship. 

The two documents listed above are the only items needed for the formal Visiting Student Program Application.  Other documentation may be needed after students accept the scheduled clerkship.

• Complete the application form and indicate your clerkship choices in order of preference, including which period you prefer for each elective.  The upload feature toward the end of the form will be used to attach the required pre-approval and completed Dean's Statement Form. 
• The application form must be received by the associated quarter's application deadline. 
• Students will be scheduled on a first come first served basis, based on the application form submission date.

If there is a spot available in the clerkship visiting students have been pre-approved for, they will be sent an email of their acceptance by the notificiation date listed on the Application & Clerkship Period Dates page based on the academic year.  The notification email will include the link to our Acceptance Form should students wish to accept the scheduled clerkship.

Waitlist notifications will be sent to those who we are unable to schedule due to no open spots. 

Students should follow the instructions on the Acceptance Form for providing requested information and accepting the scheduled clerkship.  The upload feature at the end of the form will be used to attach the required information.  

The Acceptance Form must be received by the deadline date listed on the acceptance notification.  
 

Acceptance Form:  

• Students should upload a copy of their mask fit certification to show that they have successfully passed the Respiratory Mask Fit Test within 12 months of the end of their scheduled clerkship.  See dates below. 
  
  2025-26 Academic Year Mask Fit Test: 
  -Summer Quarter - no older than September 2024
  -Autumn Quarter - no older than December 2024
  -Winter Quarter - no older than March 2025
  -Spring Quarter - no older than June 2025
  
  
  The acceptance notification will include instructions for providing medical compliance information (i.e., for medical clearance).   Students should read through the information carefully and be prepared to provide the following proof of required immunizations and tests.: 
  
  DO NOT submit immunization information/documents with the Acceptance Form.  The information must be submitted through the university's health portal.  

-MMR (Measles, Mumps, Rubella): 2 doses of live vaccines or laboratory evidence of immunity

-Varicella: 2 doses of live vaccines or laboratory evidence of immunity

-Hepatitis B: 3 doess of vaccines, or laboratory evidence of immunity from vaccination

-Tdap (Tetanus + Diptheria + Pertussis): Tdap booster (booster without Pertussis is not compliant), must be within past 10 years from the last day of the quarter

-Flu vaccine: Required seasonally - October thru March

-Tuberculin testing - TB testing completed within 12 months prior to end of clerkship, with laboratory evidence of negative QuantiFERON Gold or T-spot blood test.  PPD is not accepted.  

-If history of a positive TB skin or blood test, a chest x-ray completed within 12 months of start of the clerkship accompanied by positive TB test results. 

• At the time students are accepting their scheduled clerkship, instructions for paying the non-refundable $200 application fee will be provided.  
• Payment of the application fee is required to complete the application process.

All eligibility requirements must be completed prior to the start of the scheduled clerkship.  

Students will be compliant with many of the requirements based on their Dean's Statement Form.  However, if updated trainings and/or tests are needed to meet our requirements, students must do so in advance of their scheduled clerkship.  

If updated documentation is available at the time the Acceptance Form is submitted, students can upload the documents to the form.  If requirements are still missing after the Acceptance Form is submitted, students will receive email reminders with instructions on how to submit updated documentation.  

Students must start the medical clearance process as soon as possible.  Updated vaccinations and/or tests may be required.  It is imperative that students obtain clearance prior to their start date to avoid any delays in their participation in the clerkship.  NOTE: Immunization information/documents should NOT be submitted with the Acceptance Form.  They must be submitted via the university health portal.  Detailed information will be included in the acceptance notification.  

Visiting student applications for the 2025-26 Academic Year are accepted only during the date range listed below.  The link to the applicable quarter's application form will become available on the first date of the application period indicated below.  View the Application & Clerkship Period Dates for full details.

Identify elective rotations of interest by reviewing the list of clerkships found in the Course Bulletin.

Follow the visiting student instructions for obtaining the department's pre-approval provided in the course catalog's clerkship description.  Also review details of available elective clerkships and obtain required pre-approvals as listed on the Clerkship Descriptions & Pre-approvals page.

A pre-approval is required for each elective clerkship that is selected on the application form.
