Program Evaluation Process
Are you a first-time user of SYDCP? If yes, you may want to do a formal evaluation (left branch of flow chart) of how things went. This is very important to demonstrate "value added" for your community, sponsors, and funders and will help you build sustainability. This does require that someone in your organization ensures that all participants complete the surveys that they are sent and that you can analyze the data.
In our Access to Materials Document, we share the survey instruments we developed including those for the high school students which show changes in health knowledge and healthy behaviors.
After the first use, you only may want to collect satisfaction surveys which are the minimum amount of data needed to know whether your program provided value.
