Dr. Ehteshami Afshar is a Clinical Assistant Professor in the Division of Pulmonary, Allergy, and Critical Care Medicine at Stanford University. She earned her MSc from the University of British Columbia, specializing in health economics, before completing her residency in Internal Medicine at Yale New Haven Hospital. She furthered her training at Stanford University, pursuing fellowships in Pulmonary and Critical Care Medicine as well as Sleep Medicine. Dr. Ehteshami Afshar’s clinical expertise lies in the management of complex pulmonary conditions and sleep-related respiratory disorders, with a particular focus on home mechanical ventilation.

Dr. Fortmann's interests include population-level (community) prevention of cardiovascular disease, the epidemiology and prevention of chronic diseases, and the effects of the built environment on health. He has conducted research projects addressing tobacco use cessation, tobacco control policy, the role of retail marketing on youth tobacco use, nutrition education, blood pressure control, and lipid disorders.

My major research interests and activities over the next several years will focus on the development and evaluation of the objective measurement of physical activity in free-living populations using a variety of sensing devices and mobile phones for data collection and processing. Also, I will continue to direct the Stanford Heart Network with the major mission being to assist community-based CVD prevention/treatment programs implement more effective heart attack and stroke prevention programs.

1) The cost-effectiveness of new cardiovascular technologies.
Example: tests to screen asymptomatic patients for left ventricular systolic dysfunction.

2) Interventions to improve the quality of care of patients with heart disease. Examples: include clinical reminders and home monitoring.

3) Outcomes research using existing clinical and administrative datasets.

4) Use of echocardiography to predict prognosis (e.g. diastolic dysfunction).

Research Interests
My main research work is in "outcomes research", especially examining the field of cardiovascular medicine. Particular areas of interest are the integration of economic and quality of life data into randomized clinical trials, evidence-based medicine, decision models, and cost-effectiveness analysis. I am also interested in the application of novel genetic, biomarker, and imaging tests to assess risk and guide clinical management of coronary artery disease.

Dr. Maron is the Co-Chair and Principal Investigator of the ISCHEMIA trial, and Co-Chair of the ISCHEMIA-CKD trial. These large, international, NIH-funded studies will determine whether an initial invasive strategy of cardiac catheterization and revascularization plus optimal medical therapy will reduce cardiovascular events in patients with and without chronic kidney disease and at least moderate ischemia compared to an initial conservative strategy of optimal medical therapy alone.
