Problems and opportunities abound in the world of primary care. Stanford Medicine’s year-long Humanwide pilot project sought solutions for concerns, such as physician burnout, while tapping the potential of technological advancements, like digital health devices.
The resulting model combined cutting-edge tools of biomedicine with a data-driven, team-based approach, bringing to life Stanford Medicine’s Precision Health vision of predicting, preventing and treating disease precisely. Based at Stanford’s Santa Clara clinic, Humanwide helped 50 diverse patients address their individual risks and chart a path to achieving their personal health goals.
