Stanford and San Mateo County Heart To Heart Project

Heart attack is the number one cause of death in the United States. Stroke is the number three cause of death. Across all cultures, at least one in five adults living in the US has heart disease, stroke, or another form of cardiovascular disease. If heart attacks and strokes were eliminated, life expectancy would increase by almost seven years. Past studies show that the risk of a heart attack is much higher in people with risk factors usch as high cholesterol, high blood pressure, smoking, obesity, physical inactivity, and diabetes. The Stanford and San Mateo County Heart to Heart Project is an innovative programs that targets patients receiving health care the the San Mateo County Medical Center . Participants are seen by nurses and dietitians who specialize in heart disease prevention and treatment to reduce their cardiovascular disease risk factors. This project utilizes and seeks to enhance the Stanford Health Education and Risk Reduction Training (HEAR2T) cardiovascular risk reduction techniques.
