August 2023 Referral updates
- Please use Staff Messaging and SmartPhrases to place referrals to OT, PT, Speech (for any insurance, any location).
- Referrals to LPCH (other than OT, PT, or Speech) should be entered as CERM Referrals in OCHIN Epic.
- Referrals to SCVMC should be entered in OCHIN Epic. See details below.
- E-consults are available at LPCH for Dermatology, Endocrinology, GI, Adolescent Medicine & Adolescent GYN, and Psychiatry. Place orders in SCH Epic.
Before placing a referral - check the following
LPCH Referrals
Referrals to LPCH are ONLY appropriate for patients with Health Plan of San Mateo, Anthem Blue Cross, or state Medi-cal.
SCVMC Referrals
Referrals to SCVMC are appropriate for patients with Santa Clara Famly Health Plan, Valley Health Plan, or Santa Clara County Anthem Blue Cross
