Welcome message from the Medical Director of the PCPH Community Partnership Program

Whether you are a community member interested in partnering clinically with our primary care programs, a learner interested in working in an underserved primary care setting, a researcher looking for opportunities in community-based participatory research, or a healthcare provider interested in providing primary care in academic and community-based settings, you have come to the right place!

Our division’s community partnership program was launched in 2017 when our faculty collectively identified community engagement as a top priority. Since then, we have developed strong partnerships with several local community health clinic partners with a broad scope of programs that span clinical care, medical direction, education, quality improvement, leadership development, and research. In all of our work, we have focused our efforts on community-identified needs. And while things can get a little messy sometimes, we believe that relationship and trust require time and commitment. With the support of our key partners at Stanford Healthcare, we are fully committed to building and sustaining this important work of advancing health equity in partnership with our community.

We look forward to speaking to you about ways we can better serve our communities together.

In partnership,

David Chang, MD, DrPH

Medical Director of Community Partnerships
