The Stanford Asynchronous Virtual Co-PCP (SAV-PCP)

The Stanford Asynchronous Virtual Co-PCP (SAV-PCP) is an innovative care delivery program where primary care providers work collaboratively with the care team in order to address and respond to incoming patient messages in the electronic health record. are given dedicated time to review and respond to incoming patient electronic health record messages to co-manage patients. We aim to improve health system utilization, patient access, and clinician wellness through an asynchronous care delivery model. 

Our vision is to be a nationally recognized leading asynchronous primary care delivery program that supports the health system, our primary care colleagues, and most importantly our patients.
