How To Get Connected
The inpatient palliative care consult service is available to patients who are admitted to Stanford Hospital. In order to ensure that we work collaboratively with our colleagues, we ask that the "primary team" of providers contact us directly. The "primary team" is the group of clinicians who oversee an admitted patient's care. If you are unsure of who your "primary team" is, please ask your bedside nurse, and they can help you identify them.
If you feel that you would benefit from a palliative care consult, please let your bedside nurse or "primary team" providers know that you would like to meet with palliative care, and they can contact us.
Ask For a Referral
Ask your doctor or nurse to help you get a referral while you are admitted to the hospital.
Meet Our Team
Once your other doctors place a referral, you will meet with one of the members of our inpatient team.
Follow Up
We are also available to see you outside of the hospital in one of our 5 outpatient clinics.
