The Byers Eye Institute at Stanford is dedicated to combating blindness and preserving sight. We deliver an effective, integrated collection of comprehensive vision care specialties from one state-of-the-art, patient-centered facility in Palo Alto, California
Created to accommodate the science of complex ophthalmic medicine, the Byers Eye Institute is comprised of multiple care centers, where doctors from all ophthalmic subspecialists see patients.

For Providers: Make a Referral
The Stanford Referral Center is staffed Monday through Friday from 8 a.m. to 5 p.m, (PST). Referral specialists are available to help referrers access physicians and services at Stanford Health Care.

Physicians outside of Stanford Health Care can download the Byers Eye Institute Referral Form below and fax it to 650-320-9443 or email it to referralcenter@stanfordhealthcare.org to make a patient referral.
