Request for Course Approval - If you'd like to take a course not listed above, use this form to request approval. Requests must be submitted before the course begins. 

Individual Development Plan - Access instructions and forms here. Meetings must be scheduled by June 1 and completed by August 1 annually.

Qualifying Exam Committee Approval Form - Due by December 15

Qualifying Exam Certification Form - Take this form with you to your exam. At the conclusion of the exam, have it signed by all members and return it to the program office. 

Students apply for Doctoral Candidacy following the successful completion of the Qualifying Exam. Students must be approved for Doctoral Candidacy by the end of the second year. For details on this process please refer to the Program Handbook.

Review the Stanford Graduate Academic Policies and Procedures  Handbook (GAP 4.8.1) and the Program Handbook to make sure your committee meets university and program requirements. 

Documents required to form your Doctoral Dissertation Reading Committee (a.k.a. Thesis Committee)

3. Petition for Non-Academic Council Doctoral Committee Members (University Form) - Only required if any of your committee members are not on Academic Council

Thesis Committee Meetings: 
Have your committee chair complete this online Thesis Committee Meeting Evaluation Form after each committee meeting. 

• Take this Green Light Meeting Form to your meeting, complete it with your committee, and submit it to your Student Services Officer after it's been signed. See the Program Handbook for more information. 

Refer to the Stanford Graduate Academic Policies and Procedures Handbook (GAP), and the Neurosciences Program Handbook for policies, procedures, and deadlines related to the thesis defense. 

Oral Examination Scheduling and Approval Online Form - Submit this online form at least 30 days prior to your desired Oral Examination (thesis defense) date. On the form you will be asked to provide information about your thesis defense, submit the Oral Examination Form and your one page abstract; it is also where you request approval for a hybrid or virtual Exam. 

University Oral Examination Form - Complete this form and upload it (without Program Director signature) to the online Oral Examination Scheduling and Approval Form linked above. Note: for information about selecting your Out-of-Department Oral Examination Chair, see: Stanford Graduate Academic Policies and Procesures (GAP 4.7.1).

Application to Graduate in Axess - See (a) Graduation - Registrar's Office and (b) Applying to Graduate

Petition for Graduate Student Graduation Quarter (Axess eForm) - Students who will defend their thesis and submit their dissertation in their final quarter, OR have already defended their thesis and only have to finish writing the dissertation, may be eligible for a reduced tuition rate. The deadline to submit this is the first day of the desired quarter. Please contact program staff for more information.

Commencement - Students who apply to graduate in Axess for Spring quarter are automatically added to the SOM Commencement list.  Information is usually sent out in April. The event is coordinated by the MD Program's Office of Student Affairs. Students who are not graduating in Spring quarter but would like to participate in the SOM Commencement Ceremony must complete the School of Medicine Walkthrough Petition Form and submit it to the MD Office of Student Affairs. Instructions are updated each winter/spring quarter. 

Statement of Completion of Ph.D. Degree - See instructions here then submit a HelpSU Ticket to the "Records Unit" in the Office of the University Registrar.

General Expenses

Expense Reimbursement Form - Use this form to be reimbursed for pre-approved program expenses such as food and supplies. Submit this form, with receipts and attendee lists for meals, to Amanda Rashid. 

F31 Institutional Allowance Request Form - Submit this form to Kalai Diamond prior to making any purchases. The purpose of this form is to notify the program in advance of your plans to use your NIH F31 Institutional Allowance. With this information, we can advise you on relevant policies and procedures. 

Neurosciences IDP Mental Health Support Request Form - Use this form to submit a request to the Neurosciences IDP Mental Health Support Fund.  

Travel Expenses

Prior to making any bookings: If using your program travel funds, submit the Student Travel Certification Form to Amanda Rashid prior to making any reservations or purchases. If using your F31 fellowship, submit the Student Travel Certification Form to Kalai Diamond prior to making any reservations or purchases.  

Post Travel: Submit the Neurosciences IDP Travel Expense Reimbursement Form - with your receipts and other required documents - to Amanda Rashid within 7 days of returning from travel. Use this form to be reimbursed for program-funded travel (1st and 2nd years) or travel using your NIH F31 fellowship. 

Biosciences Financial Support

Biosciences Travel Grant - PhD students in the Biosciences can apply for awards to present research papers or posters at meetings organized by professional societies, universities, institutes, or other academic organizations. The OGE Travel Grant will award up to $1,000 per awardee once per academic year. OGE Travel Grant applications will be reviewed quarterly.

Biosciences Hardship Program - Funds from this program are intended to assist with addressing the needs of students who are experiencing a financial hardship related to the cost of living in the San Francisco Bay Area or other factors. Students who apply to the Biosciences Hardship Program are also expected to also take advantage of other University resources.

Biosciences Mental Health Support Program - Funds from this program are intended to assist with copays associated with your mental health care.
