Certified Diploma Requests for State Licensure

Some state medical boards, including California, require a certified diploma for licensure. The alumni must provide a copy of their own diploma with their licensure request, since the Registrar's office does not have copies of diplomas. The original diploma can be scanned or photographed and submitted using the Registrar's Office Request Form. The copy will be verified by the SoM Registrar's office for the licensure application. If the original diploma cannot be located, please see options below for replacement or certified electronic diplomas.

Replacement Diplomas

Stanford University will not issue duplicate diplomas under any circumstances. This measure is taken to protect Alumni from identity theft. If your diploma was permanently lost or destroyed, a replacement diploma may be ordered from the Office of the University Registrar

Stanford CeDiplomas

Stanford University provides a Certified Electronic Diploma (CeDiploma) to all graduates since Autumn Quarter 2014-15, in addition to traditional paper diplomas.  The CeDiploma can be obtained via the Office of the University Registrar's CeDiploma webpage.
