School of Medicine Registrar's Office USMLE® Information

To apply for a United States Medical Licensing Examination ® (USMLE®) Step 1 or Step 2 CK Exams:

  1. Register for NBME Licensing Exam Services (NLES)

  2. Complete the online application. Be prepared to select an eligibility period (three month window to take the exam) and pay for the exam during the online application process.

  3. At the end of the online application, there are instructions for completing a “Certification of Identification and Authorization Form” (CIF). Download the CIF, attach your photo in the indicated location, complete the Applicant Agreement section, and enter your signature and date. NBME requires a signature on the form that reasonably matches your handwritten signature.

  4. Submit the form by including it as a PDF attachment through the SoM Registrar's Office Request Form or send it as an email attachment to medregistrar@stanford.edu with "Secure:" in the subject line.

  5. The form will be completed by the SoM Registrar’s Office within 1 week. You will be notified by email when it is submitted to the NBME.

  6. Once NBME processes the CIF and the SoM Registrar’s Office verifies your enrollment, the NBME will send an email confirmation with a scheduling permit to schedule an exam date and location through Promteric.

Applications for Step 1 and Step 2 exams require an online enrollment verification on the NBME website by the School of Medicine Registrar. The online verifications are completed at least once a week on Wednesdays.
