School of Medicine Registrar's Office Licensing & FSMB Information

State Medical Board Licensing Forms

Submit state medical licensure forms to the School of Medicine Registrar's Office by using our Request Form.

Please carefully review form instructions since requirements vary between states.  

  • California:  
  1. When requesting Certificate of Medical Education form completion, please be prepared to provide your CA application number and/or file number, a copy of your diploma, and your official transcript.  
    1. If you do not have an electronic copy of your diploma, please request a copy by using CeDiploma via the Stanford MyLocker website. Either request a copy to be sent directly to medregistrar@stanford.edu, or upload the file when using our Request Form.
    2. Request an Official Electronic Transcript to be sent directly to medregistrar@stanford.edu, or upload the file when submitting your request form.
  2. Upload the partially completed form and supporting documents when you submit your Request Form.  
  3. If it will take/took more than 4 years to receive your MD, indicate needing an additional flexible curriculum explanation letter when submitting your request form. 
  • Massachusetts:  
  1. Fill in the Waiver for Release of Information section on Form A.  
  2. Include Form B if you have not graduated yet.  
  3. Upload the partially completed form(s) when you submit your Request Form.  
  4. If it will take/took more than 4 years to receive your MD, indicate needing an additional flexible curriculum explanation letter when submitting your request form.  
  5. For current students, Form B will be completed and mailed to the MA Medical Board during Spring quarter, after all graduation requirements are verified.
  • Texas:  
  1. Fill in the Applicant Section on Form D.
  2. Upload your partially completed form with a recent photo when you submit your Request Form.


For all other states, carefully follow provided instructions and submit the form by attaching it with your Request Form.

Indicate the desired delivery method for completed forms on your Request Form.  The Registrar's Office will complete licensure form requests within 1-2 weeks after submission, please plan accordingly.  An email notification will be sent to you upon completion.

Federation of State Medical Boards (FSMB)

For alumni using the Federation of State Medical Boards (FSMB) Federation Credentials Verification Service (FCVS), complete the FCVS application, including uploading a copy of your MD diploma and submitting an electronic transcript to FCVS.

Graduates from 2016 onward will have electronic diplomas and electronic transcripts on file at the FSMB if the graduate opted to send the electronic documents to FSMB when applying to graduate.
