The San Mateo Medical Center is our care delievery partner and federally qualified health center clinic. Their mission is to ”partner with our community to provide excellent healthcare for patients, including those experiencing social, environmental, or economic challenges.” As part of San Mateo County Health, the medical center serves the healthcare needs of all residents of San Mateo County, with an emphasis on education and prevention.
Community Health Center Network
The Community Health Center Network is our care delievery partner and federally qualified health center clinic. CHCN is committed to excellent, affordable care for underserved communities of the East Bay. They are a non-profit Medi-Cal managed care organization, providing business administrative support for community health centers to focus on what matters most–patients.
Center for Independence of Individuals with Disabilities
The Center for Independence of Individuals with Disabilities is a private, nonprofit corporation located in San Mateo, California. Incorporated in 1979, CID is a consumer-driven, community-based, services and advocacy organization serving San Mateo County and beyond. By federal mandate, the majority of Staff, Management, and Board of Directors consists of people with disabilities.
Roots Community Health
Founded in Oakland, California, the mission of Roots Community Health Center is to uplift those impacted by systemic inequities and poverty. They accomplish this through medical and behavioral health care, health navigation, workforce enterprises, housing, outreach, and advocacy.
