NEW PATIENT REFERRAL: If you would like to refer a patient to our ILD clinic, please fax records to (650) 320-9443, or contact our New Patient Care Coordinator at (650) 725-7061. Please leave a message and we will contact you.

Urgent Phone Consult: If you are a pulmonary physician who would like to talk to one of the physicians in the ILD program regarding an urgent consult or another patient related matter, please leave your direct/mobile number (not a general clinic/hospital phone no.) after the message and one of our physicians will call you back.

List of Records Needed: View our referral form that includes a list of all of the requested records.

If you would like be seen as a new patient at the Stanford Interstitial Lung Diseases Clinic, please have your doctor fax your records to (650) 320-9443. You may send your own records as well, and may contact our New Patient Coordinator at (650) 725-7061. Please leave a message and we will contact you.
