Trial to evaluate whether a blood pressure treatment during pregnancy to achieve targets that are recommended for non-pregnant reproductive age adults (<140/90 mmHg) compared to usual care (no treatment unless BP is severe) is effective and safe.

Objectives:  Trial to evaluate the safety and effectiveness of the MobiusHD System in patients with resistant hypertension (uncontrolled hypertension on 3-5 blood pressure medications, including a calcium channel blocker, diuretic and ACE-inhibitor or angiotensin receptor blocker). [t1] 

Type of trial: double-blind, sham-controlled, multicenter clinical trial intended to support a premarket application for the MobiusHD System.  The MobiusHD device is a self-expanding nitinol implant that is delivered intravascularly to the internal carotid sinus via the delivery catheter.

Interventions: MobiusHD Implantation versus Sham Implantation

Follow-up: All randomized subjects followed for 5 years post-implantation.

Principal Investigator: David Lee, MD, Interventional Cardiology

Trial for patients on one or no medications with SBP 150-180 mmHg - this trial is designed to see if renal denervation can be used as a primary option for hypertension (HTN).

Follow up: Patients in the SPYRAL HTN-OFF MED study will be followed every 2 weeks for 3 months then at 6, 12, 24, and 36 months.

Renal Denervation: The procedure is called renal denervation, which uses a medical device catheter to deliver low-level radiofrequency (RF) energy through the wall of a blood vessel (renal artery) to the kidney.  This procedure may decrease the kidneys’ release of hormones that raises the blood pressure, which may in turn decrease high blood pressure.

Trial for patients with hypertension (HTN) on 3 meds or more who have SBP 150-180 mmHg – the 3 drugs are specific and we can titrate their meds to see if they qualify (Ca blocker/diuretic and ACE/ARB).

Looking for patients with early-onset hypertension (i.e. less than 40 years of age) in whom secondary causes have been ruled out clinically.

Inclusion/Exclusion criteria:

Patients may be taking anti-hypertensive medications. No restrictions on level of blood pressure or type of antihypertensive medications.

Patients would need to complete only one visit and a blood test (to obtain iPSCs for reprogramming to endothelial cells)

This large, randomized multi-center clinical trial showed cardiovascular and mortality benefit for lower blood pressure, and members of our Center were site investigators for this trial. Currently, ancillary studies from this trial are being investigated at Stanford.
