Distribution of WiCell hESC Lines to Stanford Researchers:

Stanford and WiCell have signed a Memorandum of Understanding (MOU) under which WiCell can transfer human embryonic stem cells to Stanford researchers. This MOU also allows Stanford researchers to distribute WiCell lines to their fellow Stanford researchers. In order for a Stanford researcher to provide WiCell lines to you, you need to complete, sign (and if you are not a Principal Investigator, have your PI sign) and submit the following forms:

1. Stanford Incoming MTA Routing Form.  Please complete and sign (or have your PI sign) and then fax/email to the Stanford Industrial Contracts Office (ICO) at 725-7295 or ico@otlmail.stanford.edu. This form provides information ICO needs about your research. Download

2.  WiCell Simple Letter Agreement (SLA).  Please complete and sign (or have your PI sign) TWO SLAs for EACH line requested.  Please send originals to ICO. WiCell requires this form for each cell line you receive. Download

3.  WiCell Annual Certification.  Please complete and sign (or have your PI sign) and send original to ICO. WiCell requires this certification. Download

 

After ICO has received your forms and confirmed with the SCRO Manager that your protocol using the line has been approved, it will sign the SLA and send it back to WiCell, along with the Annual Certification form.  Once ICO receives the fully executed SLA back from WiCell, it will notify the Institute that you may receive the line and send you a copy of the SLA.


Distribution of Stanford hESC Lines to Stanford Researchers:

Stanford researchers do not need to complete an MTA prior to receipt of the Stanford hESC lines. However, their Stanford Stem Cell Tracking Form (http://med.stanford.edu/rmg/forms.html) must be updated to include the Stanford lines.
