PCPH Affiliate Program

Purpose of Program

Through the Primary Care and Population Health (PCPH) Affiliate Program, we welcome collaborators worldwide to engage across Stanford’s Division of PCPH (e.g., health technology companies, community-based organizations, not-for-profit organizations, healthcare delivery companies, etc.). Together, we bridge research, education, and practice to create meaningful impact for patients and communities, leveraging the breadth of over 200 faculty and multiple programs and centers across the division.

The Stanford PCPH Affiliate Program seeks to:

  • Partner with leading organizations to explore, evaluate, and scale impactful solutions that improve care delivery and patient outcomes.

  • Leverage Stanford’s multidisciplinary expertise to translate evidence into practice and foster the next generation of healthcare innovation.

  • Build a diverse network of affiliates committed to shaping the future of primary care and population health worldwide.

  • Provide affiliates with access to Stanford’s faculty, programs, and clinical settings to co-develop strategies that address pressing health challenges.
