Certificate of Insurance Verification

Professional liability and general liability insurance coverage for house staff is managed by the Stanford Risk Management Office.

To verify malpractice, obtain claims history or general liability insurance verification for house staff send an email to the Stanford Office of Risk Management. Please allow five(5) business days for a response after your emails.

All Away Rotation malpractice insurance requests to other institutions during house staff’s Stanford Health Care appointment must be approved by the GME office and submitted to Stanford Risk Management by the GME office.   • Mark your email subject line “Coverage Request”
  • In the body of your email, provide your:
    1. Full name
    2. Training program name
    3. Appointment start and end dates
    4. Reason for the request e.g. new job, etc.

Restrictions:

  • House staff are cautioned that SUMIT will not cover activities which have not been assigned as part of their Stanford training program, e.g., moonlighting at other institutions.
  • Malpractice coverage for non-standard off-site elective rotations must be provided by the host institution and not Stanford.
  • All elective rotation malpractice insurance coverage requests must be requested through the GME office.
