Outgoing/Graduating House Staff Malpractice Insurance Request
Outgoing &/or Graduating House Staff can obtain a copy of their malpractice insurance for the duration of their Stanford Health Care appointment by taking the following steps:
- Send an email to Stanford Health Care Risk Management at riskmanagement@stanfordhealthcare.org
- Mark your email subject line “Coverage Inquiry”
- Include your full name
- Appointment start & end dates
- Training program name
A copy of your malpractice insurance for the duration of the House Staff’s training will be emailed to the requester within 5-10 business days.

Rotations/Educational Activities outside of Stanford Health Care (SHC)
- Rotation/Elective malpractice insurance should be covered by the host institution & not SHC.
- In special circumstances where this is not feasible, a malpractice insurance request must be submitted by the Stanford program at the time of the elective request to Stanford GME for approval.
- The Stanford program must provide the following information for these special circumstances where malpractice is requested by SHC:
- House Staff’s full legal name
- Stanford Program’s name
- Host Institution’s name & address
- Rotation’s start & end dates
- Host Institution’s full contact name, email & phone #
- Stanford Program Administrator’s name, email & phone #
- Once Stanford GME has approved the elective & the malpractice insurance, the GME department will forward the request to SHC Risk Management for processin
- The Risk Management team will generate the malpractice request & send a copy of the malpractice to the host institutions’ contact with a copy to the Stanford Program Administrator within 5-10 business days.
- Malpractice insurance requests sent directly to SHC Risk Management without GME’s approval will be returned.
