All Away Rotation malpractice insurance requests to other institutions during house staff’s Stanford Health Care appointment must be approved by the GME office and submitted to Stanford Risk Management by the GME office.

• Trainee name
• Site
• Dates of rotation
• Site director name, email, phone number
• Address of site
