Alameda County Recipe4Health (R4H)

The Alameda County Recipe4 Health (R4H) initiative is an integrative model for healthcare that addresses the social determinants of health (specifically food insecurity and social isolation) and chronic disease. This model advances health and racial equity by transforming the healthcare system’s capacity to increase access and utilization of affordable, nutrient-rich, whole medically supportive food and to provide ongoing behavioral change support to improve the overall health of communities. The R4H program began in Fall 2019 with funding from the U.S. Department of Agricultural Gus Schumacher Nutrition Incentive Program (USDA GusNIP).

The overall goal of the R4H project is to evaluate the effectiveness of the All-In Alameda produce-prescription program in increasing consumption of fruits and vegetables, reducing individual and household food insecurity, improving health outcomes and healthcare utilization.

Open Source Wellness

OSW is a Community and a "Behavioral Pharmacy." Four basic practices, underlying physical and psychological health, are facilitated to treat individuals in need.

University of California San Francisco

UCSF is one of the nation’s leading academic medical centers connecting world-class research, education, and patient care.

In partnership with Dig Deep Farms, the food farmacy intervention includes 16 weekly prescriptions for fresh produce. Each delivery contains approximately 16 servings and is intended to supplement a patient's regular groceries for the chance to incorporate new produce into their diet with no financial risk. These foods include whole grains and vegetables. With the on-going pandemic, boxes of produce are delivered straight to the patient's homes. 

Dig Deep Farms promotes sustainable and regenerative practices to cultivate nutrient-rich foods to distribute to their community.
