Student Forms
Year One
- Plan your courses using the PhD Planning Worksheet
- Complete a BMP Lab Rotation Form at the start of each rotation (three total)
- Have Faculty Preceptors complete an evaluation form at the end of each rotation: BMP Laboratory Rotation Evaluation Form
- Complete the BMP Thesis Lab Choice Form by July 1.
- Apply for outside fellowships
Year Two
- Pass Qualifying Examination
- After passing quals: submit the Application for Candidacy for Doctoral Degree
- Apply for outside fellowships
Years Three and Four
- Submit the Reading Committee form to sydmalki@stanford.edu no later than one year after passing your qualifying exam.
- Meet annually with your Doctoral Dissertation Reading Committee.
- Apply for TGR Status
