Habit Tracker

Instructions: Print out the handout and check off each box when a habit is completed. The numbers at the top of the tracker represent the days of the month (e.g. 1 = 1st day of the month). Place the tracker somewhere visible, like the fridge or bathroom mirror so that you don't forget about it!

Bladder Diary

Instructions: Print out this Bladder Diary. It has columns for tracking your child's water intake, pooping, peeing, and any accidents. Refer to the number on the left side of the diary to note these details at different times throughout the day. Bring this diary to your next medical appointment!
