Arbor Free Clinic Medical Records:
A patient, or their representative, may obtain a copy of medical records, or have copies of medical records sent to another facility.
Please submit all Cardinal Free Clinics (Pacific Free Clinic or Arbor Free Clinic) medical records requests and subpoenas to cardinalfree@stanford.edu. Upon receiving your email, a staff member will respond to your request and follow up as needed.
