Fax patient records to (650) 498-7452

Medical records should include:

1. Recent labs (including but not limited to serum free light chains, SPEP, UPEP)

2. Pathology reports from all biopsies related to the amyloid diagnosis

3. Recent clinical notes from any doctor involved with the amyloid diagnosis

4. Copy of echocardiogram report (if applicable).
