WHAT: The Stanford Health Care Memory Support Program provides support for adults and families facing memory and cognitive impairment. Through a single point of contact, the Memory Support Program offers consultations, assessments, family meetings, home visits, and care coordination of medical and non-medical services. The program team can develop a comprehensive care plan customized to meet individualized needs. The care plan includes caregiver support, education, care approaches, and connections to community resources.

WHAT: The Northern California and Northern Nevada Chapter is one of over 70 Alzheimer's Association chapters serving communities across the United States. It includes eleven offices in two states and offers a 24/7 helpline, one-on-one care consultations, and support groups.

WHAT: The ADEAR Center is a U.S. government-funded resource. It strives to be a current, comprehensive, unbiased source of information about Alzheimer's disease. ADEAR provides information about causes, treatment, cures, and diagnostic tools, which has been reviewed by National Institute on Aging scientists for accuracy and integrity.
