Administrative Information for Entities
Department and entities are responsible for providing ADMINISTRATIVE INFORMATION to the Health and Safety Programs Office and updating contacts as necessary. Accurate contact information is essential for communication and is the first step in implementing TRACE in the School of Medicine.
- Information on the TRACE Health and Safety Verification Program was presented at the March DFA Meeting.
- Key contact information, provided in an MS Word document, will be sent out by email to entities by March 31. (Template is available) Please make sure that an individual is identified in the following roles:
- Health and Safety Contact (H&S Contact)
- Emergency Coordinator
- DFA, Division Manager or Administrative Mgr/Director
- Department/entity will return updated/verified form by April 15.
- Our office will contact the Health and Safety Contact for three groups by May 1st to begin reviewing departmental programs for Training, Compliance and Emergency Management records.
