Emergency Notification for Laboratories
The special topic for Quarter 1 is Emergency Notification for Laboratories
, where we will review information posted in the life safety box, along with notification, reporting and responding to emergencies situations or work-related accidents. Please take this opportunity to look at questions that came up during the University-wide emergency exercise.

- Chemical Inventory Update is an annual process where labs review and update information in the ChemTracker database. The University submits a summary of the entered information to regulatory and response agencies on an annual basis and whenever there is a renovation project in the building. The same information is posted in the Life Safety Box for use in case of an emergency.
- Emergency Notification Form lists current emergency contact information for department/lab contacts. PI's and other contacts will be notified when an emergency or incident occurs in their areas. Key contacts do change during the year; is the information posted outside your chemical storage areas accurate and up-to-date?
- Chemical Storage Map is maintained and updated by the laboratory showing location or room hazards, large pieces of equipment safety equipment and storage location for chemical, radiological or biological hazards. Don't forget to date/initial the Emergency Notification Form and Chemical Storage Map; this will document that the forms have been reviewed!
EMERGENCY NOTIFICATION AND REPORTING

- Do you have your School of Medicine Information Card? It is important to carry this card with you at all times. Check with your Department Emergency Coordinator if you need a new card.
- The card summarizes what to do in case of an emergency and includes Department Hotline information, Emergency Assembly Point (EAP) and Personal Preparedness Information.
- Did you know the SOM Information Hotline: (650) 723-7233 (3-SAFE)?
- Make sure to write in the phone number for your Department Emergency Hotline.
- Check online for additional School and Department Emergency Information Hotlines, How to Get Emergency Information and information on ALERT-SU on the University EHS Website.
- Emergency Preparedness information is also available on the Health and Safety Programs Website.
- Review the information covered in the Q3-07 Special Topic, Individual Emergency Preparedness, especially the Emergency Orientation checklist.
RECEIVING TREATMENT FOR WORK-RELATED ACCIDENTS
- Steps for reporting Work-related Accidents, Incidents & Exposures of Employees
- Inform supervisor.
- Call Occupational Health Center: (650) 725-5308 if event involves a work-related injury or illness.
- Complete an SU-17 form, available at: http://su-17.stanford.edu
- Additional services are available through the new Stanford Occupational Health Center for Stanford employees.
- Vaden Health Center provides students and Post-Doctoral Fellows assistance for Medical Emergencies. (Life-threatenting Emergency, Stanford Emergency Services)
- Emergency Orientation - Checklist covers basic life safety elements that everyone should be familiar with. It should also allow a supervisor the opportunity to go over any other Department/Division/Institute specific emergency information.
- Emergency Instruction Card, including a Physician version, provides critical emergency instructions and phone numbers. Contact the Health and Safety Programs office (650.723.0110) for additional cards.
