Good Medical Practice and the Law:
Physician Documentation Checklist Offered

Clinical documentation improves the safety and even efficacy of clinical care, and in any case, failure to comply with rules results in potentially costly and disruptive disciplinary action for the hospital and/or caregivers, notes Larry Shuer, chief of staff.

As part of ongoing efforts to improve clinical documentation and adhere to external requirements, Medical Staff Services has prepared the following checklist for physicians to review.

Shuer noted that the next JCAHO [Joint Commission on the Accreditation of Healthcare Organizations] accreditation site visit is expected in April 2001, and documentation is reviewed from at least the prior year. The items on this checklist focus on issues which have commonly been cited by JCAHO and other regulatory agencies during surveys at other hospitals.
