Innovations in Adolescent Transitional Health Care Grants

Internal funding opportunity for all Stanford faculty, Instructors, employees, residents, medical fellows, graduate students, postdoctoral fellows and all Packard Children's Hospital, Stanford Children's Health & Stanford Hospital employees and medical staff from all disciplines of patient care are invited to submit applications.

Internal deadline: Monday, March 23, 2015, 5 p.m. (see application guidelines below)
Grant recipients announced: April 10, 2015
Funds will be available starting May 1, 2015 for a 12 month period.

Amount of funding:

Grants will be awarded in amounts up to $25,000. Total Costs must include the 8% infrastructure charge (ISC) taxed by Stanford University on all gift funds


Eligibility


Purpose:

This is the inaugural year for the Innovations in Adolescent Transitional Health Care Grants. The funding is made possible through the James Gallagher Patell Fund in Transitional Medical Care. The funding committee welcomes submission of ideas for improving transitional patient care that represent departures from conventional thinking. Grant applications that directly impact on improvement in the quality of life of adolescent and young adults during this health care transition will be given special consideration. Applications from multidisciplinary groups are also encouraged. Grants with focus on improving transitional care for patients with congenital cardiac diseases will be given special consideration.
