Session Description

This session will review events in which system errors have occurred and resulted in harm to patients.

Education Goals

  • Educate pertaining to the types of errors that can occur in our health system and encourage care providers to speak out if the encounter situations where they believe there is a potential error occurring
  • Educate our care givers about action plans that have been put in place in response to serious safety events, to decrease the likelihood of such events from occurring again
  • To encourage a culture of transparency and improvement
