Training and Licensing Fees for Stanford Self-Management Programs

Training Fees
Training at Stanford University
Costs for training at Stanford are through 2008 are (training fees will be raised January 1, 2009):
$1500 per health professional
$800 for a lay person with chronic disease
for the 4½ day CDSMP or 5 day combination Tomando Control de su Salud/Diabetes Master Training. Included in this training is all instruction, one set of all materials, and breakfast for 5 days, lunch for 4 days (5 for Spanish combination training) and one evening banquet.
Cross Training (1 day) for either Positive Self-Management or Arthritis Self-Management is $350. Included is all instruction, one breakfast and one lunch as well as one set of all materials.
Cross Training for Tomando Control de Su Salud (1½ days) is $450. Included is all instruction, 1 breakfast and 2 lunches (or 2 breakfasts and 1 lunch) as well as one set of all materials.
Cross Training for combination Tomando Control de su Salud and Diabetes (2 days) is $500. Included is all instruction, 2 breakfasts and 2 lunches, as well as one set of all materials.
Off-site Training by Stanford University
Fees for 4½ day CDSMP, Spanish CDSMP, Spanish Diabetes, or PSMP Master Training through 2008 (fees will be raised on Janauary 1, 2009):
- $9,000 paid to Stanford
- $6,000 paid directly to 2 trainers ($3,000 each)
Additional cost if 1½ day Tomando cross-training is offered during the same period:
- $3,300 paid to Stanford
- $1,200 paid directly to 2 trainers ($600 each)
- $1,800 paid to Stanford
- $1,200 paid directly to 2 trainers ($600 each)
Fees for stand-alone cross trainings
Cross training is offered only to those previously trained as below.
CDSMP to Spanish CDSMP cross-training (2 days)
- $3,600 paid to Stanford
- $2,400 paid directly to 2 trainers ($1,200 each)
Spanish CDSMP to Spanish Diabetes (1.5 days)
- $3,600 paid to Stanford
- $2,400 paid directly to 2 trainers ($1,200 each)
CDSMP to PSMP (1 day)
- $1,800 paid to Stanford
- $1,200 paid directly to 2 trainers ($600 each)
Arthritis to CDSMP (2 days)
- $3,600 paid to Stanford
- $2,400 paid directly to 2 trainers ($1,200 each)
Additional Fees
The organization contracting for the training will need to supply:
- Transportation and per diem for two trainers
- Materials for all trainees. These include one book for each program, two manuals for each program, and two relaxation CDs. The master copies for the manuals will be sent to the organization for reproduction.
- One full set of completed workshop charts for each program
- One copy of the CDSMP provider video (for CDSMP training)
- Training in Spanish for either arthritis or chronic disease has one additional exercise CD

Licensing Fees
Every organization offering a Stanford program must have a license for that program. If more than one program is being offered, the organization must have a license for each program offered.
Each license if good for 3 years from the date of issue. After each three year interval, licenses must be renewed.
Every licensed organization must make a yearly report (due on the anniversary of original license agreement) to Stanford stating the number of workshops and number of participants offered during the past year, number of leader trainings conducted, and the names of all leaders trained with contact information.
- Licenses for organizations receiving training from Stanford will be discounted by $250 dollars. Each organization can apply only one discount per program.
- Licenses for organizations not receiving training from Stanford will be at the following rates:
- $500.00 for offering 10 or fewer workshops a year
- $800 for offering 20 or fewer workshops a year
- $1000 for offering 30 or fewer workshop a year
- If an organization is offering more than 30 workshops a year, it must negotiate a license with Stanford’s Office of Technology Licensing
If in the yearly report an organization has offered more workshops than it is licensed for, it should remit the difference in licensing fee per the chart above along with the report. If a licensee does not submit a report, it will be considered to be in breach of the agreement and subject to termination.
All licenses must be paid for and signed before an organization receives training. It is the responsibility of the Master Trainers or T-Trainers offering the training to see that all licenses are in order before training.
The license has been designed to be printed and submitted as is. It has been greatly simplified and has been written using plain English language with as little legalese so as to be understood by the lay person. The low cost and volume of these licenses make it impossible for us to negotiate special clauses for each organization. If your organization needs custom agreement, the cost of the license will increase by a minimum of $1,000 USD.
