Patients should please send or bring with them the following:

  • Copies of your medical records/discharge summary/physician notes
  • A copy of your x rays
  • A copy of your lab reports
  • If you are presently under medications, bring them with you to your appointment

The Pituitary Clinical Center is located in the Neurosciences North Clinic, third floor of the Boswell Clinic Building, Stanford Hospital, 300 Pasteur Dr., Stanford, CA 94305.
