New Patients
While patients are welcome to refer themselves, we encourage your regular doctor to contact us so we can resolve issues or logistical problems that cause delays in seeing a neurosurgeon.
Appointments are scheduled after a Nurse Practitioner has reviewed relevant medical records and diagnostic studies in order to match patients with the doctor best suited to the patient's needs.
You may fax all of your pertinent records and diagnostic and radiology reports to our secure New Patient Fax Line: (650) 723-6002.
You will be contacted within 2-4 working days to schedule an appointment. The more information we have, the faster it will be for us to designate a doctor and contact you for an appointment.
Referral Status: If you have not heard from us within 72 hours regarding your referral, please telephone the new patient coordinator at (650) 725-5792.
After an appointment has been scheduled, new patients are sent a packet that contains:
- A Pre-appointment Check List
- Payment Policy and Insurance Information
- Maps and Directions
- A 6-page Health History Form
Please make sure all information is complete.
Medical Records
You will be asked to bring copies of your medical records and imaging studies (X-rays, MRI, and CT scans) with you on the day of your appointment. Your first visit could last 2-3 hours because several doctors may review your medical records and discuss your condition with you. It is often helpful to write down a list of questions that you may have and to bring along a family member or friend to this session.
