An Email Form Template You Can Use

To use this email form template, you will need two files: "feedback.html" and "thankyou.html." Feedback is the actual email form. Thankyou is the page the user is sent to after clicking the "Submit" button. Note: This form will only work on the med.stanford.edu server. If your site is hosted on another server, you may still be able to use it by setting it up on the med.stanford.edu server and linking to it from your site.

Here's what you need to do:

Download the two html files
Replace the banner graphic with your own, or with text.
Change the text on both pages to something appropriate for this page on your site.
Change the "recipient" values within the form to ones appropriate to your group.
If you want to have a "Thank You" page, you will need to add an invisible "redirect" input and use the absolute URL (i.e. http://med.stanford.edu/etc/etc...) of your thankyou page.
Upload the pages and graphics (if any) to your site.

Don't forget to request approval from IRT, if you don't already have a working FormMail form.
