Lecture Videotape Permission to Use

Permission forms must be on file for all EdTech videotaped lectures/presentations that are being posted to the web.

Secure Site:

This secure form should be signed and forwarded to EdTech for any taped video that will be posted to a secure, password-protected website (i.e. the CWP Curriculum Portal).

Public Site:

This public form should be signed and forwareded to EdTech for any taped video that will be posted to a public website (i.e. departmental website that does require a password to access).
