Referring Physicians

Thank you for considering involving us in the care of your patient. We appreciate your commitment to ensuring your patients receive outstanding care throughout pregnancy.

The Stanford IBD and Family Program is a collaboration between specialists in gastroenterology, IBD, maternal fetal medicine, reproductive endocrinology & fertility and nutrition. We operate as a concurrent care model and value communication with the patient's primary care team throughout the different stages of pregnancy. Management of patients with IBD during pregnancy can be complex, with patients utilizing care from primary care physicians, emergency room providers, gastroenterologists and obstetricians. We welcome referrals from all medical specialties and encourage our patients to continue close follow-up with their primary providers. 

To initiate the referral process

  • from within Stanford: place a referral to gastroenterology and type "IBD and Family Clinic" in the comments section.
  • from outside of Stanford:

  2. ONLINE: click on the link below to initiate a referral through the PRISM system. The PRISM portal enables you and your clinical team to submit referrals, view status updates, access medical records, and more entirely online.
