Title: Safely Bending Trend for Medically Fragile Patients

Abstract:

While Medicare has struggled to reduce per capita health spending safely for FFS beneficiaries via its demos, managed care medical groups are succeeding primarily via intensifying care for patients with unstable major chronic illnesses. What distinguishes those that are succeeding? Does it suggest a much wider set of unharvested opportunities to safely lower health spending? What evaluation methods are best suited to evaluating such innovations in care delivery?
