COBRA Information for House Staff

IMPORTANT 2014 COBRA INFORMATION - HOUSE STAFF (PDF)

2014 COBRA ACCOUNT DETAILS (PDF)

Your benefits at Stanford Hospital & Clinics and Lucile Packard Children's Hospital will terminate on the last day of the same month as your termination date.

Within two weeks of your termination, you will receive COBRA continuation information from the Vita Administration, Inc.

You have 60 days from the notice date to sign up for coverage under COBRA. After Vita receives your application and check, coverage will be retroactive to the 1st of the month following your termination date.

COBRA packets will not be sent out prior to your termination date.

If you will be leaving the state or will have a new address immediately following your termination, it is important that you change your address, via e-Connect, prior to your termination date. This will assure that your notice will be sent to the right address. Once logged in, click on Address Change.

Rollover of TDA Accounts - Tax Implications

You are not required to rollover your Transamerica accounts, but if you choose to do so, contact Transamerica to initiate the process. Your employer (SHC) is required to confirm with our affiliated organizations (Stanford University, University HealthCare Alliance, Stanford Linear Accelerator, Sierra Camp and Alpine Chalet) that you are no longer employed at any of these organizations before releasing your funds. The University closes for Christmas break in mid-December and does not reopen until after the New Year. Transactions not completed by November 30th, 2013, will have to wait until the 1st of the following year - there may be tax implications - please check with your tax advisor.
