All participants will recieve the Qualtrics Measures links after the informed consent process.

Please keep track of your Clinician ID that the study team will give you during informed consent.

Make sure to label each measure with the following information: 

Patient ID's will be Recruitment Year (eg. 2017), 4-digit Clinician ID (eg. 1023), followed by a 3-digit Patient ID (start with the #1 and add 01, 02, 03, etc.), so “101” would be your first patient, “102” is second patient), followed by Session #. Each session will be labeled Recruitment Year-Clinician ID-Patient ID-Session #.
