Evaluation Instruments
The Division uses a wide range of evaluation instruments and tools to collect information. Two of the most common tools are the course and clerkship online survey forms.
Although we collect information using these tools throughout the School, we only collect information from required courses and clerkships at this time.
We have provided copies of these forms primarily for electives and other non-required courses:
