In-House Weaning/Separation Cage Card Request Form

The following form is used when weaning or separating animals into additional cages from in-house animal colonies. This form is not to be used for ordering animals. Please continue to use the Animal Requisition Form (SU-45) for ordering animals.

Please complete all applicable spaces and submit to the Comparative Medicine Administrative Staff. Please allow 1 (24 hours) full business working day to process cage card request.
