2017-18 CERC Fellowship Graduation
CERC recently celebrated their 2017-18 class of fellows with a graduation ceremony. This year the two interdisciplinary teams created care models focusing on "Late Life" and "High Need, High Cost" patient populations. The heart-felt ceremony included a guest speaker, Dr. Manali Patel, who was one of the fellows from CERC's first cohort.
